If a player is injured at a practice or in a game, she is covered. Players are not covered while they are driving to or from a game or practice. In order for the player to file a claim you MUST:
- Make sure that the injury is noted on a game report by the referee.
- Fill out the proper form (Accident Claim Form)
- Submit the proper documentation within 90 days of injury.
Be aware that the claim form must be signed and dated by a League officer. You must include a copy of the GAME REPORT. There is a $400.00 deductible, and the policy coordinates benefits with any other insurance coverage the player might have.
